Open Access Medical Journals in 2027: What Has Shifted Since You Last Checked
If you are drawing up a list of the best open access medical journals for 2027, the journal names are the easy part. What makes a 2027 plan succeed or fail is everything around them: whether the APC you noted last year still applies, whether your funder's access rules have moved, whether the reporting checklist your trial must follow has been revised, and whether your institution will pay the fee or expects you to. Most of these points changed between 2024 and 2026, and several will keep moving through 2027.
Our 2026 companion page listed fifteen open access medical titles by reported APC tier. This page does not repeat that list. It takes the same fifteen titles, regroups them by what an author is actually trying to achieve, and then works through the budgeting, compliance and timing decisions that matter if your paper is to appear in 2027. Where we mention a fee, it is the figure reported on our 2026 page and is marked as such. It is a reference point, not a quote for next year.
Four changes worth knowing about
Zero-embargo public access
The NIH Public Access Policy has applied since 1 July 2025. Accepted manuscripts from NIH-funded work must be publicly available in PubMed Central by the official publication date, with no 12-month delay. Other funders are watching closely.
New trial reporting standards
CONSORT 2025 (for completed randomised trials) and SPIRIT 2025 (for trial protocols) were published together in 2025. Expect editors to ask for the updated checklists rather than the 2010 and 2013 versions.
National APC support in India
India's One Nation One Subscription (ONOS) scheme, launched in January 2025, includes APC support for eligible corresponding authors in participating journals. It was set up for an initial three-year period, so check its status for 2027 on the official portal.
Clearer fee labels in directories
DOAJ now separates journals that charge no fees at all from those that charge no APC but may have other fees. That makes it easier to see the full cost of a title before you build a budget around it.
Shortlist by Goal: Our 2026 Medical Titles Regrouped for 2027
A clinician writing up an unusual case, an epidemiologist with a large cohort study and a digital-health team testing an app need different journals. Grouping titles by goal shows you where to start reading author guidelines. It is not a ranking, and none of these journals is endorsed by us. They are third-party titles that appeared on our 2026 guide, given here as data.
| Your 2027 goal | Titles from our 2026 list (ISSN as listed) | 2026 reported APC | Check first in 2027 |
|---|---|---|---|
| Case report or a no-fee route across specialties | Cureus (2168-8184) | No APC | Whether any fee now applies, and its patient-consent form |
| Methodologically sound clinical study, any field | PLOS ONE (1932-6203); Medicine (0025-7974); Scientific Reports (2045-2322) | About $2,290 with waivers; about $1,900; about $2,490 | Current APC, waiver route and data-availability rules |
| Biomedical research with published assessments | eLife (2050-084X) | About $2,500 | How its public review and assessment model now works |
| Protocols, negative results, broad clinical scope | BMJ Open (2044-6055) | About £2,163 | Fee in your currency and protocol-publication criteria |
| General medicine, translational and public health | BMC Medicine (1741-7015); Frontiers in Medicine (2296-858X); JAMA Network Open (2574-3805) | About $3,890; about $2,950; about $3,500 | Scope statements and article-type limits |
| Global health, epidemiology, health policy | PLOS Medicine (1549-1676) | About $6,460 with waivers | Waiver eligibility and pre-submission enquiry process |
| Digital health, informatics and AI in medicine | Journal of Medical Internet Research (1438-8871); The Lancet Digital Health (2589-7500) | About $2,995; about $5,900 | AI-use disclosure rules and software or code sharing |
| Diabetes and endocrinology | BMJ Open Diabetes Research & Care (2052-4897) | About £2,163 | Specialty scope and any society-member discount |
| Surgical trials and reviews | International Journal of Surgery (1743-9191) | About $3,900 | Required reporting checklist for each design |
| Clinical work within broad natural sciences | Nature Communications (2041-1723) | About $6,790 | Editorial threshold and current APC |
| Health research needing a low, fixed fee and a DOI | See our network section below | See the journal cards | Scope fit and fee payable after acceptance |
Read each row as a starting point. Open the journal's "about" and "author guidelines" pages, read three recently published articles of the type you plan to submit, and ask yourself whether your manuscript would look at home among them. If it would not, the APC is irrelevant. A desk rejection costs time, not money, and time is the scarce resource in a 2027 plan.
The 2026 Reported APC Range at a Glance
The chart below shows the 2026 reported APCs of the titles quoted in US dollars. The two BMJ titles, quoted in pounds sterling (about £2,163), are left out so that the bars use a single currency. Bar length is relative to the highest value shown. Treat this as a picture of the spread, not as 2027 prices: publishers usually review APCs every year, and waivers or agreements can bring the amount you pay down to zero.
Two points stand out. First, most of the titles sit between roughly $2,000 and $4,000, so for many authors the real choice is about scope and editorial fit, not about saving a few hundred dollars. Second, the most expensive end of the range is where waivers and institutional agreements matter most. An author who qualifies for a full waiver at a $6,000 journal pays nothing. An author who does not qualify pays more than many departments spend on a year of conference travel.
Budgeting the APC for a 2027 Medical Paper
Budgeting comes before submission, not after acceptance. Many authors discover the fee only when the acceptance letter arrives, and then either pay from their own pocket or withdraw the paper, losing months. For 2027, settle the funding question during the drafting stage. There are four realistic routes.
Grant budget
If the study is funded, check whether the award allows publication costs and whether they must be claimed before the grant end date. A paper accepted after the grant closes may not be payable from it.
Institutional or national agreement
Your library may have a read-and-publish agreement covering some journals. In India, ONOS APC support may apply to participating journals for eligible corresponding authors. Confirm the list and process at onos.gov.in and with your library.
Country-based waiver
Many publishers give full or partial waivers to authors from countries on the Research4Life Group A and Group B lists, often automatically based on the corresponding author's affiliation. The lists and rules differ by publisher.
Low fixed-fee journal
A peer-reviewed journal with a small, published fee charged only after acceptance removes most of the uncertainty. Our network journals work this way; see the network section below.
A simple budgeting worksheet
| Line | What to write down | Where the figure comes from |
|---|---|---|
| First-choice journal APC | Fee and currency on the date you check | Journal's fees page, saved as a PDF or screenshot |
| Waiver or discount | Eligible or not, and full or partial | Publisher waiver policy; written reply to your request |
| Agreement coverage | Covered, partly covered or not covered | Library, research office, ONOS portal |
| Fallback journal fee | Fee of your second choice, in case of rejection | Second journal's fees page |
| Optional extras | Language editing, figure redrawing, colour charges | Supplier quotes; journal guidelines |
| Exchange-rate margin | A buffer on any foreign-currency fee | Your finance office's guidance |
The cheapest journal is the one whose fee you knew, could pay and had approved before you clicked submit.
How to ask for a waiver properly
Waiver requests work best when they are made at submission, in writing, and before any review takes place. State your country and affiliation, say whether the work is funded, and ask for the outcome in writing. Do not wait until acceptance: many publishers will not consider a waiver once a manuscript is in production, and an editor's goodwill is not a waiver policy.
Funder, Access and Licence Rules to Re-Check for 2027
Access and licence rules decide which journals you are allowed to use, not only which ones you would like to use. Before you commit to a target for 2027, check four things against your funder's and institution's current policies.
- Deposit ruleDoes your funder require a repository copy (for NIH, PubMed Central), and by when?
- EmbargoIs any delay allowed? Under NIH's policy since July 2025, it is not.
- LicenceIs CC BY required, or would CC BY-NC be accepted?
- Data sharingMust you publish a data availability statement or deposit the data?
- RegistrationWas the trial or review registered before enrolment or screening began?
For authors outside the United States, the NIH rule applies only when the research is NIH-funded, including through a subaward to a foreign collaborator. It still matters as a signal: other funders and governments often adopt similar policies after a major funder moves. If your project has any NIH money in it, check the journal's deposit arrangements before you submit, because some PubMed Central journals deposit the final version for you while others leave it to the author.
Do
- Save your funder's open access policy as it reads on the day you submit
- Choose a licence your funder accepts, which is CC BY in most cases
- Confirm who deposits the manuscript, you or the journal
- Keep the trial registration number in the abstract
Avoid
- Assuming the 2026 rules still apply in 2027
- Signing a copyright transfer that blocks a required repository deposit
- Choosing a non-commercial licence when your funder requires CC BY
- Treating a preprint server as a substitute for a funder's repository requirement
Reporting Standards Updated in 2025 and What They Mean for 2027 Submissions
Editors at reputable medical journals use reporting guidelines as a screening tool. A missing checklist is one of the quickest routes to a desk decision. The important change for 2027 is that the two core trial guidelines were revised in 2025 and published together for the first time. SPIRIT 2025 covers what a trial protocol should contain, and CONSORT 2025 covers what a completed trial report should contain. The two are designed to be used side by side, so the protocol and the final report match.
| Study design | Guideline to follow | What an editor will look for |
|---|---|---|
| Randomised trial report | CONSORT 2025 | Updated checklist, flow diagram, registration number, protocol access |
| Trial protocol | SPIRIT 2025 | Outcomes, sample-size reasoning, data-sharing plans stated in advance |
| Systematic review or meta-analysis | PRISMA 2020 | Search strategy, flow diagram, protocol registration where possible |
| Observational study | STROBE | Clear description of the setting, participants, variables and bias |
| Case report | CARE | Written patient consent and a timeline of the case |
| Diagnostic accuracy study | STARD | Reference standard and flow of participants |
The EQUATOR Network library of reporting guidelines is the reliable place to find the current version of each checklist and its extensions. If your study is already under way and was designed against an older checklist, say so in your cover letter and complete the new checklist anyway, noting any items that cannot apply. Editors respond far better to candour than to a checklist with page numbers that do not match the manuscript.
A 2027 Calendar for Clinical and Health Authors
A medical paper submitted in mid-2027 at a selective journal may not appear until late 2027 or later. Working backwards gives the realistic schedule below. The timings are general ranges, not promises: review speed varies widely between journals and specialties, and one extra round of revision can add two months.
- October–December 2026
Confirm ethics approval and consent documentation, check trial or review registration, and decide which reporting guideline applies. Draft the target shortlist using the goal table above.
- January–February 2027
Lock the dataset, run the planned analysis, and draft tables and figures. Ask your library or research office about agreements and APC support for the coming year.
- March–April 2027
Write the full manuscript with the checklist open beside it. Run a similarity check, have co-authors approve the text, and agree the authorship order and contribution statements in writing.
- May–June 2027
Re-verify the first-choice journal's fee, scope and indexing, then submit with a cover letter, checklist and any waiver request. Post a preprint only if the journal's policy allows it.
- July–October 2027
Expect reviewer comments and at least one revision round. Reply point by point and keep a record of every change. If rejected, re-format for the next journal within two weeks.
- November–December 2027
After acceptance: pay any fee, check proofs carefully, confirm the DOI, deposit the accepted manuscript where your funder requires it, and share the published link.
If you are working to a fixed deadline, such as a degree submission, a promotion file or a grant report, count back from that date and add a buffer for one rejection. Authors who start in early 2027 with a deadline in late 2027 should plan for a fallback journal from the outset.
Decision Path: Matching a 2027 Manuscript to the Right Kind of Journal
Most choices come down to one question: does your goal require a journal in a specific index? If it does, the index decides your shortlist and the budget follows. If it does not, scope, peer review quality, cost certainty and speed decide. The diagram summarises both branches.
How to verify an index claim yourself
For PubMed and MEDLINE, search the journal's ISSN in the NLM Catalog of journals and read the "currently indexed" field. A journal whose articles appear in PubMed Central is not necessarily indexed for MEDLINE. For Scopus, search by ISSN in the official Scopus source list and check that coverage is still active. For DOAJ, search the Directory of Open Access Journals and read the fee fields. Confirm that the ISSN matches the one on the journal's site using the ISSN Portal. Four searches take only minutes and expose most false index claims.
Where to Publish a Health or Medical Paper in 2027: Our Journal Network
Not every health manuscript needs a high-APC international title. Pilot studies, community health surveys, nursing and pharmacy research, health-management studies, public-health education projects and early-career work often need something else: careful peer review, a DOI, a permanent open access record and a fee that is known in advance. That is the role our network plays.
To be clear from the start, JMVRA, TIJER and JNRID are not presented as indexed in PubMed, MEDLINE, Scopus, Web of Science or DOAJ. If your institution or funder specifically requires one of those indexes, use the verification steps above and the goal table on this page. If it does not, our journals give health authors an affordable, peer-reviewed open access option. All three are multidisciplinary, publish monthly, and accept health and medical sciences papers that fit their scope.

AI Disclosure, Data Sharing and Preprints: Trends Medical Authors Should Expect in 2027
Three trends that were already visible in 2025 and 2026 are likely to show up in more author guidelines in 2027. None is yet uniform across journals, which is why reading the specific journal's current policy matters more than any general rule.
Disclosure of AI assistance
The ICMJE recommendations, which many medical journals follow, say that AI tools cannot be listed as authors and that their use in writing or analysis should be disclosed. Expect more journals to ask what tool you used, for what purpose, and how you checked the output. Never ask an AI tool to generate references or patient data; fabricated citations are a common reason for retraction.
Data availability
Data-sharing statements are standard for clinical trials at ICMJE-member journals, and funders increasingly expect data to be deposited where privacy allows. Plan anonymisation and a data-access statement during analysis, not after acceptance.
Preprints
Medical preprint servers let authors share findings early, and many journals accept submissions that have been preprinted. Some specialty journals and some types of clinical findings call for extra caution, because an unreviewed preprint can be quoted in the media. Check the target journal's preprint policy, label the preprint clearly, and link it to the final version once published.
Mistakes to Avoid With Medical Open Access Journals in 2027
These are the errors we expect to cost health authors the most time and money next year. Each one is avoidable with a few minutes of checking.
- Copying last year's APC into a grant report or budget without checking the journal's fees page again
- Sending a manuscript to a cloned journal website that copies a real title's name and ISSN; always reach the journal through the ISSN Portal or the publisher's main site
- Treating an emailed invitation promising rapid publication as an editorial opportunity
- Submitting a trial report written to the old CONSORT checklist without explaining why
- Leaving consent forms for case reports until the journal asks for them
- Assuming PubMed Central visibility means MEDLINE indexing
- Requesting a waiver after acceptance instead of at submission
- Submitting the same manuscript to two journals at once to save time
- Ignoring your institution's list of approved or excluded journals before choosing a venue
Hijacked and cloned journal sites deserve special attention in medicine, because a well-known title name carries weight with readers and promotion committees. A cloned site may show a real ISSN, real editorial board names and a professional design. The test is the route you took to reach it: if you did not arrive from the ISSN record, the index entry or the publisher's own domain, do not submit.
How Journey & Wisdom Tools Support a 2027 Medical Publication Plan
Journey & Wisdom services sit behind our network journals and are also available to authors and journal teams. For a medical manuscript, four of them are directly relevant.
JWSCAN before you submit
Checks the manuscript for similarity using iThenticate (by Turnitin), with an overall similarity index and a source-by-source report, so you can fix overlapping text in methods and background sections before an editor sees it.
JWJMS for tracking
The journal management system behind submission: author dashboard, desk screening, reviewer invitations and reminders, revision rounds, decisions and a full record of each step.
JWDOI for a permanent link
Registers Crossref DOIs and deposits article metadata, so the published paper has a persistent identifier you can cite in reports and CVs.
ORWave for open access
Open Research Wave is the group's open-access publication arm, one of the routes through which reviewed work is made openly available.
For anything beyond these descriptions, such as a hospital or university planning its own health sciences journal, contact the team to discuss your requirement at [email protected] or on WhatsApp at +91 91063 65667.
Final Word on Choosing Open Access Medical Journals for 2027
The best open access medical journals for 2027 will be the ones that fit your manuscript, accept your licence and funder rules, and have a fee you have already arranged to pay or waive. Start from your goal, not from a famous name. Verify every index claim yourself by ISSN, budget before you submit, write to the updated reporting standards, and leave a buffer in your calendar for one rejection.
Use the goal table as a reading list, the calendar as a schedule and the decision path as a check on whether you really need a high-APC indexed title. When an affordable, peer-reviewed, DOI-backed open access venue is the right answer, our network journals are ready to read your work, and you pay only after acceptance.
How Journey & Wisdom Helps

Frequently Asked Questions
Which open access medical journals should I target in 2027?
Start from your goal rather than a ranking. Use the goal table on this page to find titles from our 2026 list that match your article type and field, then confirm each one's current APC, scope and indexing on its own site. If you need an affordable peer-reviewed venue rather than a specific index, consider JMVRA, TIJER or JNRID, where the fee is charged only after acceptance.
Will APCs at medical journals be higher in 2027?
Nobody can say for certain. Publishers usually review APCs every year, and the figures on this page are 2026 reported values, not 2027 prices. Check the fees page within a few weeks of submitting, save a dated copy, and keep an exchange-rate buffer if you pay in a foreign currency. Waivers and institutional agreements can change the amount you actually pay.
Is there a free open access medical journal for case reports?
On our 2026 guide, Cureus was reported as charging no APC and accepts case reports across specialties. Fee policies can change, so confirm its current terms on the journal's own site before submitting in 2027. Whichever journal you choose, prepare written patient consent and follow the CARE guideline, because both are commonly required for case reports.
How do I check whether a medical journal is in PubMed before I submit?
Search the journal's ISSN in the NLM Catalog and read the indexing status field. Note the difference: PubMed includes MEDLINE-indexed journals and also articles deposited in PubMed Central, so finding a paper in PubMed does not by itself prove MEDLINE indexing. Make sure the ISSN on the journal's site matches the ISSN Portal record.
Does the NIH zero-embargo policy affect authors outside the United States?
It applies when the research is funded by NIH, including through subawards to collaborators in other countries. If any NIH money supports your study, the accepted manuscript must be publicly available in PubMed Central by the publication date. If your work has no NIH funding, the policy does not bind you, but your own funder may have similar rules, so check.
Can Indian authors get an APC paid through ONOS in 2027?
ONOS includes APC support for eligible corresponding authors at participating institutions publishing in participating journals. The scheme began in January 2025 for an initial three-year period, so its terms for 2027 should be confirmed on onos.gov.in and with your library. Eligibility often depends on your primary affiliation and is usually triggered after acceptance.
What is CONSORT 2025 and does my trial paper need it?
CONSORT 2025 is the updated reporting guideline for randomised trials, published in 2025 alongside SPIRIT 2025 for trial protocols. If you are submitting a randomised trial report in 2027, expect journals to ask for the 2025 checklist and a flow diagram. Find the current version and explanation papers through the EQUATOR Network.
How early should I start preparing a medical paper for 2027 publication?
For publication in late 2027 at a selective journal, aim to submit by mid-2027, which means locking data early in the year and checking ethics, registration and reporting requirements in late 2026. Review and revision commonly take several months, and one rejection can add two or three more, so build that buffer in.
Are JMVRA, TIJER and JNRID indexed in PubMed or Scopus?
No. We do not present JMVRA, TIJER or JNRID as indexed in PubMed, MEDLINE, Scopus, Web of Science or DOAJ. They are low-cost, peer-reviewed, multidisciplinary open access journals with Crossref DOIs. If your requirement names a specific index, verify your target on that index's official site and choose accordingly.
Can I publish a health sciences paper in JMVRA?
Yes, if it fits the journal's multidisciplinary scope and meets its ethical requirements. Include ethics approval, consent statements where relevant and the reporting checklist for your design. The fee is ₹971 for Indian authors or US$41 for international authors, charged only after acceptance. Contact [email protected] if you are unsure about scope.
What should an APC waiver request include?
Make the request at submission, in writing. Give your country and institutional affiliation, say whether the research is funded and by whom, and explain briefly why you cannot pay the full fee. Ask for the decision in writing before review begins, and keep the reply with your submission records. Check the publisher's waiver policy first, as many apply waivers automatically.
Is a low-cost open access medical journal less credible than an expensive one?
Price is not a measure of quality. Credibility comes from real peer review, a clear editorial board, published policies, a verifiable ISSN, permanent identifiers and transparent fees. Some excellent journals charge nothing and some poor ones charge a lot. Judge a journal by those signals and by the articles it has already published, not by the size of its fee.
Should I post a medical preprint before submitting in 2027?
Many journals accept preprinted manuscripts, but policies differ, and some clinical findings call for caution because unreviewed results can be misreported. Read the target journal's preprint policy first, label the preprint clearly as not peer reviewed, and update it with a link to the published version. If you are unsure, ask the editor before posting.
Can I use AI tools when writing a medical manuscript?
Many journals allow limited AI help with language, provided you disclose it and take full responsibility for the content. The ICMJE recommendations say AI tools cannot be authors. Never use AI to produce references, data or images, and check every claim it suggests. Follow the specific journal's AI policy, which may be stricter than general guidance.
How do I avoid predatory or hijacked medical journals in 2027?
Reach every journal through its ISSN Portal record, its index entry or the publisher's main website, never through an emailed link. Check that the editorial board names are real and reachable, that peer review is described in detail, and that fees are published before submission. Be wary of guaranteed acceptance, publication within days, or requests for payment before review.
